We are committed to providing you with high-quality therapy services. To ensure that we can serve you and other clients effectively, please review the following practice policies.
Appointment Scheduling
Initial Appointment: Please contact our office via phone (970 805 0570) or email (clayton@coloradocreativetherapies.com) to schedule your initial appointment.Ongoing Appointments: Regular appointments are scheduled based on mutual availability. It is your responsibility to keep track of your appointment times and dates.
Cancellation and No-Show Policy
Cancellation Notice: Appointments must be canceled or rescheduled at least 24 hours in advance. You can cancel or reschedule by calling our office or sending an email.
No-Show Policy: If you do not attend a scheduled appointment and fail to notify us within the required 24-hour notice period, you will be charged the full rate for the missed session.
Late Cancellations: Appointments canceled or rescheduled with less than 24 hours' notice will incur a charge equal to the full session rate.
Exceptions: We understand that emergencies and unforeseen circumstances can occur. Please contact us as soon as possible to discuss any special circumstances.
Payment Policies
Fees: Our standard fee for therapy sessions is $150 per session unless we have agreed on another rate.Payment Methods: We accept credit cards and venmo payments.
Insurance: If applicable, we will assist you with a superbill for reimbursement through your insurance provider, but it is your responsibility to verify your insurance coverage and benefits.
Confidentiality and Privacy
Confidentiality: All information shared during therapy sessions is confidential and protected under HIPAA regulations. However, confidentiality may be breached in specific circumstances as outlined in the HIPAA Privacy and Confidentiality Agreement.
Record Keeping: We maintain records of your therapy sessions for the purpose of providing effective treatment and complying with legal requirements.
Client Responsibilities
Attendance: Regular attendance is crucial for effective therapy. If you are unable to attend a session, please notify us as soon as possible.Participation: Active participation in therapy is essential for achieving your goals. Please come prepared to discuss and work on the issues addressed in your sessions.
Communication: Inform us of any changes in your contact information, insurance details, or other relevant information.
Emergency Procedures

Termination by Client: You have the right to discontinue therapy at any time. We recommend discussing your decision with us to ensure a proper transition.Termination by Therapist: We reserve the right to terminate therapy services if:
It is determined that we are not able to meet your needs.
There are significant breaches of our policies.
No Sessions Scheduled: If no therapy sessions are scheduled for 3 consecutive weeks without prior communication from you, we may consider your case closed and terminate services.
